We have two standard forms that need to be filled out and sent back to us. Letter of Authorization: The letter of authorization allows us to go directly to the pension plan administrator and obtain accurate information to the date of marriage and the date of separation. The pension member must sign this form. Information Sheet: This form provides us with all of the required dates to value the pension. There is also a line to fill in for expected retirement income.
