Thank you for visiting our web site.  Attached are the two documents that we need you to complete to get the process started (Information Sheet and Authorization Letter).  Please complete and sign them as soon as possible and return them to us.  Also, please send us copies of the following documents, if you have them:

  1. An annual pension statement closest to your date of separation,

  2. Any correspondence from your pension administration regarding the current status of your pension,

  3. The amount of RRSP's and similar accounts that you owned on the date of separation,

  4. A copy of your member's pension and benefits handbook current to your date of separation.

It can take approximately four weeks for the pension administrators to provide us with the information required to complete our calculations.  Once we have all the information we will complete our calculations and mail the completed report to you.  Please advise as to the mailing address that you wish your report to be mailed to.

If you or your spouse are receiving your pension already we will require the following information:

  1. Income tax returns from close to the date of separation,

  2. Any additional income you or your spouse is receiving,

  3. When you plan to stop working (if you are currently),

  4. When you plan to start receiving the Canada Pension Plan,

  5. A Canada Pension Plan statement,

  6. Information about any RRSP's or similar accounts.
